This is an individual Hit Lab Membership for one active Salem Sluggers player. Membership begins October 16, 2026, after payment confirmation and a current Hit Lab Membership waiver, and the initial term ends June 30, 2027.

Monthly installments: $80.00 at signup and 8 remaining monthly payments through June 1, 2027. Pay-in-full: $720.00 at signup. The total remaining initial-term obligation is $720.00. No installments for months before signup are charged.

The membership renews automatically at $80.00 per month beginning July 1, 2027 unless canceled. Written notice by June 1, 2027 prevents July renewal; after renewal, the applicable 30-day cancellation notice applies.

Enter the parent or guardian contact information, player name and payment method. A current CYOJ Hit Lab waiver must also be on file before the player uses the membership.

After payment is confirmed, a digital CYOJ Hit Lab membership card will be issued for the enrolled player and emailed to the parent or guardian email used during checkout. The card must be presented to CYOJ Hit Lab staff at check-in for each visit in which individual membership access or membership benefits are being used. Staff may scan the card's QR code to check the player's current payment status. Staff must also verify a current Hit Lab Membership waiver before granting individual access.

I am the parent or legal guardian of the enrolled player. I have reviewed and agree to the Membership Agreement and authorize the initial and recurring charges shown.

Bank payments can take several business days to confirm. If the player does not already have a CYOJ Hit Lab waiver on file, complete the Hit Lab Membership Waiver before the player's first visit or use of this membership. When the form opens, select “Hit Lab Membership.” If a waiver is already on file from a previous visit, a new one is not required.

Renewal and cancellation

Salem Sluggers controls the team-practice schedule. Individual membership access is separate from scheduled team practices.
